To expedite patient registration and processing, please download the forms shown here. Complete the forms by typing required information into the given fields, print out form, sign, date and bring them with you to your appointment. You can also email them to our secure email at [email protected] or fax them to (225) 636-5547, along with a front and back copy of the patient’s insurance card.
